Covid-19 Release Waiver

Due to the outbreak of the Coronavirus (COVID-19), our business is taking extra precautions with the care of every customer as we have implemented enhanced sanitation/disinfecting procedures in compliance with CDC guidance. Symptoms of COVID-19 include:

  • Fever
  • Fatigue
  • Dry Cough
  • Difficulty Breathing

I agree to the following:

  • I understand the above symptoms and affirm that I, as well as all attendees participating within G-Code Game Truck, do not currently have, nor have experienced the symptoms listed above within the last 14 days.
  • I affirm that I, as well as all attendees participating within G-Code Game Truck, have not been diagnosed with COVID-19 within the past 30 days.
  • I affirm that I, as well as all attendees participating within G-Code Game Truck, have not knowingly been exposed to anyone diagnosed with COVID-19 within the past 30 days.
  • I affirm that I, as well as all attendees participating within G-Code Game Truck, have not traveled outside of the country or to any city considered to be a “hot spot” for COVID-19 infections within the past 30-days.
  • I understand that G-Code LLC cannot be held liable for any exposure to the COVID-19 virus caused by misinformation on this form or the health history provided by each client.

Our business is following theses enhanced procedures to prevent the spread of COVID-19:

  • Masks must be worn at all time for all individuals when inside of G-Code Game Truck.
  • Hand Sanitizer readily available during all parties/events within G-Code Game Truck.
  • Deep thorough cleaning of the Game Truck and controllers before and after events.

By checking the box or booking with G-Code LLC, I agree to each statement above and release G-Code LLC from any and all liability for unintentional exposure or harm due to COVID-19. I agree to the following:

  • I understand the above symptoms and affirm that I, as well as all attendees participating within G-Code Game Truck, do not currently have, nor have experienced the symptoms listed above within the last 14 days.​
  • I affirm that I, as well as all attendees participating within G-Code Game Truck, have not been diagnosed with COVID-19 within the past 30 days.
  • I affirm that I, as well as all attendees participating within G-Code Game Truck, have not knowingly been exposed to anyone diagnosed with COVID-19 within the past 30 days.
  • I affirm that I, as well as all attendees participating within G-Code Game Truck , have not traveled outside of the country or to any city considered to be a “hot spot” for COVID-19 infections within the past 30-days.

​I further acknowledge that G-Code LLC can not guarantee that attendees participating within G-Code Game Truck will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, G-Code staff, and other G-Code clients and their families.

I hereby release and agree to hold G-Code LLC harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the G-Code Game Truck, or that may otherwise arise in any way in connection with any services received from G-Code LLC. I understand that this release discharges G-Code LLC from any liability or claim that I, my heirs, or any personal representatives may have against the Game Truck with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from G-Code LLC. This liability waiver and release extends to the Game Truck together with all owners, partners, and employees.
